Abstract:

Heinrich, in Domino theory, showed the human role in accident causation. In recent decades, this role has been lnore cleared due to mechanical guards and workplace phys~cal condition improvements. In spite of adequate inspections, procedures as well as ergonomic considerations, human error is still the main cause of accidents. Safety professionals have therefore focused on safety culture. Many studies have shown that improving safety culture involves two major points: 1) Recognition and evaluation of all aspects of safety culture, and 2) Being aware that cultural changes are very time -consuming phenomenon.
