Investigation of Human Faults and the results of it in Esfahan Steel Industires

Abstract:

Human Errors and Consequence Analysis in a Control Room - Isfahan Iron Melting Factory - IRAN Developing of modem technology reduced number of workers on hazardous workplaces. Although by stress on human roles induced increase of human errors. In recent decades studies has shown that the main causes of accidents is human and hisher unsafe Acts. Today consequences of a human emr may lead to catastrophic accident in international dimension. Iran is not an exception from this rule. This study carried out based on HEIST (Human Enor Identification in System Tools). Surry decision and Rose&Rose models was used to classification of decision steps and types of human error prospectively. The result of this study was showed the occurrence of numerous human errors on mentioned task is possible and its consequences could led to serious manpower and economical losses. Finally based on Rasmossen theory on behavior, In order to decrease human errors among operaators,it is recommended to perfonn training course for all operaators based on behavior safety principles.
