Recurrent urogenital myiasis caused by Psychoda Spp. in a teenager: A case report

Myiasis is an uncommon parasitic infestation of live human tissue with dipteran larvae of varied species. When fly eggs have access to urethra, urogenital myiasis could appear by hatching the eggs and movement of larvae upward the urogenital tract. There is a strong association between poor hygiene and myasis. In this case, a 14 year-old female diagnosed with recurring urogenital myiasis caused by Psychoda Spp. at the first admission. The patient was a teenage student girl lived in an old home in central part of Isfahan city. The patient had no physical or mental risk factor and seemed in a normal personal hygienic condition. The patient complained of abdominal pain, sever genital pruritus, painful and frequent urination and seeing moving larvae in urine. A mild hematuria was reported in her random urine analysis and three live larvae were observed in her first morning urine sample. The larvae were identified as Psychoda spp. The symptoms were disappeared after a period of using urinary tract antiseptic and antibiotic therapy. The patient returned to hospital after a four month period with the same complaints as seen in the first admission. Psychoda larvae were detected and identified in her urine sample again. After a precise question it revealed that not only her bath room hygienic condition is not appropriate but also she had used to hang up and dry her underwear in that condition where the fly could lay their eggs on her underwear. The bath room disinfection was taught to the patient and her family and a course of urinary tract antiseptic and antibiotic therapy was prescribed for her. The patient recovered completely after a week. It seems that increasing the public knowledge and teaching the problem in schools may prevent or decrease the occurrence of similar cases in future.
