Effects of dehydroepiandrosterone (DHEA) on response of ovarian stimulation in IVF cycle in infertile women with diminished ovarian reserve: Before and after clinical trials

Background: One of the best therapies in women with diminished ovarian reserve (DOR), is the useof dihydroepiandrosterone (DHEA) before starting in vitro fertilization (IVF) cycles to increase ovarianreserve.Objective: In this study, we aimed to compare the effect of DHEA on women with DOR below and above35 years old and to see if it’s useful at an earlier age.Materials and Methods: This is a clinical trial study performed on 35 infertile women with DOR whoreferred to Imam Khomeini Hospital in Sari in 2017. The intervention was for six weeks, DHEA tabletswere used before the start of the ICSI-ET cycle. Antral follicular counts (AFC) and serum AMH levels weremeasured before and after the intervention and then ICSI was performed. Changes in AMH levels andAFC, number and quality of ovum and embryos and pregnancy, and implantation and abortion rateswere measured.Results: Results: The mean AMH levels (p=0.02) and AFC (p<0.001) after DHEA consumption weresignificantly different from those prior to administration of DHEA, and the increase in the AFC was moresignificant in the age group under 35 (p=0.03). Also, these changes were more significant in Body massless than 25 kg/m2 (p=0.04).Conclusion: Conclusions: It can be concluded that probably the supplementation of DHEA in womenwith insufficient ovarian capacity improves IVF prognosis and other parameters including Antralfollicular counts and AMH especially in women under 35. Studies have shown that the probabilityof pregnancy occurrence in women was lower with increased BMI.
