The effect of splenectomy on red cell alloimmunization in thalassemia patients from Iran

Background: RBC transfusion is the main therapeutic option in thalassemia patients, but alloimmunization against red cell antigens is the major complication of transfusion in chronically-transfused patients. Splenectomy is typically performed in many patients with thalassemia to alleviate hypersplenism or stabilize transfusion requirements. There is controversy association between splenectomy and alloimmunization. However in most studies the sample size is very small and the age of splenectomy was not considered. In the present study we evaluate the association of splenectomy and alloimmunization in Iranian thalassemia patients referred to Tehran Adult Thalassemia Clinic between 2016 and 2018.Methods: Retrospective study from 2016 to 2018 was conducted on 1276 Iranian thalassemia patients transfused regularly in Tehran Adult Thalassemia Clinic and Clinical and transfusion records of patients were analyzed.Results: records of 1276 thalassemia patients (650 male and 626 female) with a median age of 21±9.43 (range: 1–61) were analized. Five hundred and sixty eight patients (44.5%) in this study had undergone splenectomy and the median age of splenectomy was 11.4±7.44 (range: 1-45). Red cell alloantibodies were reported in 151 (11.83%) patients and significant association between splenectomy in any age with either alloimmunization or number of alloantibodies were not observed.Conclusion: Results of present study demonstrated that there was no association between splenectomy and alloimmunization in Iranian thalassemia patients. So, however splenectomy may associate with some adverse effect in thalassemia patients that should be taken into account when considering this surgery but splenectomy isn’t a risk factor for alloimmunization in thalassemia
