Introduction: Cancer as the second leading cause of death has always been a threat to human life and due to its peculiar nature and lack of a favorable treatment, it is considered as a research priority in all communities. Colorectal cancer is the third most common cancer in Iran. Previous studies, such as the Women’s Health Initiative, have shown that a low dose of vitamin did not protect against colorectal cancer. But the dose–response relationship has not been adequately studied. Higher intake of calcium and vitamin D has been associated with a reduced risk of colorectal cancer in epidemiologic studies and polyp recurrence in polyp-prevention trials. However, randomized-trial evidence that calcium with vitamin D supplementation is beneficial in the primary prevention of colorectal cancer is lacking. The increasing trend of colorectal cancer incidence in Iran and the close relationship with the Low plasma vitamin D are the underlying reasons for this study. In this review we describe the mechanisms by which Vitamin D3 are thought to have antineoplastic activity and review published preclinical and clinical studies that support anti-colorectal cancer activity and we try to show association between deficiency of Vitamin D3 in population of Iran and increase colorectal cancer incidence.Methods: A comprehensive literature search was performed in PubMed, Scopus, Science Direct and GOOGLE Scholar to identify primary research reporting the effects of Vitamin D3, through October 2016. Two authors independently reviewed and selected eligible evidences. Finally, we combined the results of previous investigations and carried out heterogeneity and sensitivity analyses on them.Results: Several articles and studies have proposed Vitamin D3 have chemopreventive effects on many cancers. Likewise, many mechanistic studies show that the active vitamin D metabolite (1α, 25-dihydroxyvitamin D3 or calcitriol) inhibits proliferation and promotes epithelial differentiation of human colon carcinoma cell lines that express vitamin D receptor (VDR) via the regulation of a high number of genes. A key action underlining this effect is the multilevel inhibition of the Wnt/β-catenin signaling pathway, whose abnormal activation in colon epithelial cells initiates and promotes CRC.Conclusion: This study provides evidences which represent increasing level of vitamin D3 can be effective in preventing of colorectal cancer. Thus, vitamin D3 supplements would be probably an appropriate adjuvant therapy. As well as, in this study we showed vitamin D deficiency in the Iranian population can be effective in increasing colorectal cancer in this country. Nevertheless, more research is recommended for this aim.
