Comparing Hospital Effectiveness of Tabriz Obstetrics and Gynaecology Hospitals Managed as Autonomous and Budgetary Hospital

Objective: This study was aimed to compare organizational effectiveness of autonomous hospital and budgetary ones.Methods: A longitudinal, retrospective study was conducted to compare effectiveness of gynaecology hospitals of Tabriz University of medical sciences during 2010-2012. First we selected criteria related to hospital effectiveness through a literature review, and then through AHP technique we have weighed each factor according to experts viewpoints. Information about patient satisfaction, employee satisfaction, safety, equipment management, quality management, average length of stay, bed occupancy rate, and bed turnover ratio gathered. Finally a composite variable calculated as total effectiveness score. Independent Samples T test and χ2 were conducted to compare effectiveness score between hospitals.Results: Among 8 variables, safety was identified as the most important indicator and medical equipment was the least one. Total effectiveness score of autonomous hospital (31.79%) comparing to effectiveness score of budgetary hospital (28.33%) highlights more effectiveness in autonomous hospital (P=0.790).Conclusion: Despite the recent emphasis of health ministry toward deploying autonomous hospital in Iran, we didn’t find significant differences between organizational effectiveness of autonomous and budgetary hospitals, therefore it is necessary for policy makers to control the implementation of hospital autonomy instructions by hospital managers in order to ensure that they are successful in achieving desired goals otherwise they should revise the relevant guidelines.
