Arachnoid cysts are cerebrospinal fluid-filled sacs that are located between the brain or spinalcord and the arachnoid membrane, one of the three membranes that cover the brain and spinalcord. Primary arachnoid cysts are present at birth and are the result of developmentalabnormalities in the brain and spinal cord that arise during the early weeks of gestation.Secondary arachnoid cysts are not as common as primary cysts and develop as a result ofhead injury, meningitis, or tumors, or as a complication of brain surgery.Typical symptoms of an arachnoid cyst around the brain include headache, nausea andvomiting, seizures, hearing and visual disturbances, vertigo, and difficulties with balance andwalking. Diagnosis usually involves a brain scan or spine scan using diffusion-weighted MRI(magnetic resonance imaging) which helps distinguish fluid-filled arachnoid cysts from othertypes of cysts.The arachnoid cyst of the optic nerve is avery rare entity. Some appear to be isolatedcongenital anomalies, whereas others are associated with optic nerve sheath meningiomas. Itsdifferential diagnosis from the primary optic nerve glioma is difficult and important to bemade in order to be treated properly.We report a 49 year- old man with progressive proptosis and decreased vision of the left eye,imaging revealed the presence of a cystic lesion around the optic nerve. Pathologicalexamination showed a cyst wall lined by cuboidal to flattened meningothelial cells over alayer of fibrocollagenous tissue. IHC staining showed positive reaction for EMA andnegative CD31, CK in overlying epithelial cells, GFAP staining in stromal spindle cells wasnegative.Conclusion:Arachnoid cysts of the optic nerve are benign, slowly progressive conditions that may resultin proptosis and visual field changes. The clinicopathologic features of arachnoid cyst shouldbe differentiated from optic nerve sheath meningioma and other conditions.
