Melanotic neuroectodermal tumor of infancy (MNTI) is a rare, usually benign neoplasm of neural crest origin. MNTI behaves more aggressively than a usual benign tumor and is able to acting as a malignant neoplasm (6.5% of cases). This neoplasm containing primitive cells producing Melanin. At present, 92.8% of MNTIs occurred in the neck and head region of infants during the first year of life. The most common location of tumor is anterior maxilla( 68%-80% of cases). However the best treatment for MNTI is surgical resection, local recurrence rate is between 10% and 60%. Pre-operation computed tomography (CT) scanning of the head and neck region is used to evaluate the content and the extent of MNTI. This tumor has special pathologic and imaging features, and most be considered in differential diagnosis with other head and neck masses. In this report, we present a rare case of MNTI in a 6-month-old child in the Maxilla and discuss the clinical and histopathologic features of this neoplasm. After tomography and biopsy, the diagnosis of MNTI was established. Histological examination indicated a remarkable combination of neural, melanocytic, and epithelial cell differentiation. One year after tumor excision, the child has had no recurrence.
