Abstract: Mammary analog secretory carcinoma was first proposed as a unique salivary gland tumor entity in 2010 by Skalova and colleagues that showed morphologic, immunophenotypic and molecular overlap with secretory carcinoma of the breast. Prompted by their report, this is the first case report of MASC identified in Iran occurring in the parotid gland of a 65 years old male patient with an ETV6-NTRK3 gene translocation. Histologically, the tumor was composed of cells with eosinophilic cytoplasm, ovoid nuclei and centrally located nucleoli and formed microcystic spaces with intra-luminal eosinophilic secretions which were periodic acid-schiff-positive. Immunohistochemically, the tumor cells were positive for mammaglobin and S100 protein. This tumor was formerly described as acinic cell carcinoma, the main differential diagnosis of MASC which does not exhibit ETV6 rearrangement.
