Primary neuroendocrine (NE) carcinoma of the breast is included in the latest WHO classification under the category of NE tumors. Primary neuroendocrine carcinoma of the breast is a rare entity, comprising <1% of breast carcinomas. Pure NE carcinoma of the breast with classic morphological and phenotypic features is rare. Described here is the case of a 70-year-old woman who developed an invasive tumor in the left breast. Solid component measuring 5.5 cm x 6 cm x 8.5 cm. The tumor was composed of only endocrine elements in the invasive part. Microscopic observation of serial sections preparation revealed malignant neoplasm with neuroendocrine differentiation signified by the presence of cells with round to ovoid relatively monomorphic nuclei with nuclear grooves and eosinophilic cytoplasm arranged in solid nests with organoid patter, small acini and rosettes and micro papillary structures. The prognosis of neuroendocrine carcinomas is thought to be worse than invasive mammary carcinomas, Therefore, immunohistochemistry for neuroendocrine markers is important in the routine practice to prevent overlooking neuroendocrine carcinomas.
