Background: Placental mesenchymal dysplasia (PMD) is a rare placental abnormality resembling partial hydatidiform mole. It is characterized by enlarged placenta having grapelike vesicles which may be mistaken for partial molar placenta in ultrasonography and pathological examination Material and method (Case presentation): We report the occurrence of PMD in a 20- year-old primigravida who developed eclampsia at 28 weeks gestation and delivered a normal female infant weighing 1230 gr. Results: Gross examination of the placenta revealed grape-like vesicles and normal villi occupying the maternal surface of the placenta. Microscopically abnormal stem villi containing loose stroma and muscularized fetal vessels with cistern formation were seen with no evidence of trophoblastic proliferation and inclusions in the chorionic villi. The mother and the infant showed no related disorder during three years of follow-up. Conclusion: Distinction between PMD and molar pregnancies is important as the former is compatible with fetal life and termination of the pregnancy is not necessary.
