The challenge of pancreatic neoplasm therapy by using mesenchymal stem cell: A systematic review

Background and Objectives Pancreatic neoplasm has a dismal prognosis due to late diagnosis and early metastatic spread. Even in patients subjected to the surgical removal of the cancerous lesions and intensive chemo/radiotherapy, the overall survival remains very low. Mesenchymal stem cells have attracted significant attention in cancer research as a result of their accessibility, tumor-oriented homing capacity, and the feasibility of autotransplantation. This review provides a comprehensive overview of current challenges in pancreatic neoplasm therapy. Method We performed a comprehensive electronic search on the PubMed and Science direct for all studies of Pancreatic neoplasm using Pancreatic neoplasm and Mesenchymal stem cell keyword between 2010 to 2016. These search keyword were confirmed with a MeSH database. Out of 208 studies from PubMed and 815 studies from Science direct , 25 study that met our primary criteria of interest were selected, also Unofficial sources were used. In order to assess the quality of the initial studies, the form developed by the researcher was used. Results The studies show that in a Pancreatic neoplasm micro-environment, MSCs are believed to play both, a protumorigenic and an anti-tumorigenic role. However, this is dependent on a host of factors like, types of MSCs, its source, in vivo or in vitro conditions, factors secreted by MSCs and interactions between MSCs, host’s immune cells and cancer cells. Among several cytokines secreted by MSCs, TRAIL (Tumor necrosis factor related apoptosis inducing ligand) is reported to be pro-apoptotic for tumor cells. Conclusion Stem cell therapy using MSCs show great potential as improve for Pancreatic neoplasm. However, there are many issues and limitations that need to be resolved.
