malignant transformation in oral lichen planus,report of a case and review of literature

Introduction: lichen planus is a common mucocutaneous disease with a prevalence rate of 2%. It seems that immunologic responses are involved in incidence of the disease. Most patients are middle aged adults and women are more likely to develop lichen planus. Oral mucosa usually occurs bilaterally. Although buccal mucosa is the most common site, but it can also affect lingual, labial, gingiva and palatal mucosa. It occurs as several forms such as reticular, plaque form, atrophic, erosive and bullous. Reticular type is the most common form. OLP is associated with an increased risk in oral squamous cell carcinoma (OSCC) Malignant transformation rate has been reported from 0.65% to 6.5%. and malignant transformation is most probable in erosive and atrophic formsObjective: We present a case with malignant transformation.Case presentation: 54-year-old male with a 22-year history of OLP was referred to department of oral pathology at Tehran University of Medical Sciences. Clinical examination revealed bilateral atrophic involvement of tongue, reticular form of OLP on right buccal mucosa with mild pigmentation, and a relatively broad area of erosive OLP on left buccal mucosa. Incisional biopsy was performed and SCC was diagnosed.Conclusion: Patients with OLP usually display lesions with distinctive clinical morphology and characteristic distribution. In addition, the disease is presented with a confusing array of forms and the patterns is mimicking other diseases. Since patients with OLP may be at an increased risk of the development of squamous cell carcinoma, therefore, periodic follow-up is mandatory to detect malignant transformation.
