Blood and bone marrow eosinophilia in patients with Hodgkin s lymphoma, the study of 80 cases in Shiraz

Blood eosinophilia occurs in different disorder including allergic, parasite, fungal, bacterial infections as well as rheumatoid arthritis and vascular disorder. Blood eosinophilia in cancer is a rare phenomenon reported in a relatively small number of cases. Hodgkin s lymphoma is an exception, in which eosinophilia occurs at a frequency of about 15%. The purpose of this study is to determine the rate of bone marrow and blood eosinophilia in 80 patients with Hodgkin s lymphoma and its possible role in prognosis in relation to its subtypes.Materials and MethodIn this cross-sectional study, records of 108 patients with a pathologic diagnosis of Hodgkin s lymphoma during five years in Shiraz university-affiliated hospitals are the material of this study. 14 patients were excluded, because of the common parasitic disease in the age group and possible eosinophilia due to the parasite. Also, another 14 patients had no subtype classification, so were excluded from the study. Average eosinophilia count of each patient was recorded. More than 500 absolute eosinophilia per cubic millimeter in peripheral blood and more than 6% eosinophil in the bone marrow differential count were considered as eosinophilia.ResultsFrom 80 patients, 13 patients accompanied with eosinophilia (16.25%). The majority of patients with blood eosinophilia had either mixed cellularity (10 patients, 77%) or nodular sclerosis (2 patients, 15%). All of them appear in the young age group (10-19 years, six patients, 46%) and (20-29 years, seven patients, 54%). Ten patients (77%) were male three patients (23%) were female. (male/female: 3/1). However, the overall prognosis was better for the eosinophilic patients, in which the subgroup of Hodgkin s lymphoma was mixed cellularity and nodular sclerosis. Bone marrow eosinophilia in this study is indeed more common than blood eosinophilia and occurring in 18 patients out of 80 patients with Hodgkin s lymphoma(22.5%).ConclusionBlood and bone marrow eosinophilia is seen in a relatively significant number of Hodgkin s lymphoma in Shiraz. It is more common in subtypes of mixed cellularity and nodular sclerosis than others. These two subtypes of Hodgkin s lymphoma have a better prognosis than other subtypes. So eosinophilia should be considered as a good prognostic sign in patients with Hodgkin s lymphoma. However, more investigation is needed to determine its precise prognostic value.
