Metastatic germ cell tumor to breast: A case report

Background: Testicular germ cell tumors are a heterogeneous group of malignancies with diverse pathological pattern and clinical symptoms and manifestations. Type II of these tumors is usually seen in adults. However, metastasis is rare; in some case, it may be seen with the catastrophic outcome. Here were report a metastatic case of testicular germ cell tumors in a young adult patient.Case Presentation: The patient was 31-year-old male subject with pain and bulging in right testis leading to radical orchidectomy two years ago with a diagnosis of mature teratoma. The patient had no follow-up and was attended after two years with DVT symptoms and a mass in the right breast. In chest and abdominal CT scan, there were multiple nodules and abdominal lymphadenopathy. In laboratory assessment, there was high Beta-HCG (125900 IU/ml), BUN (29 mg/dl), and creatinine (2.5 mg/dl) accompanied with leukocytosis (18900/mm3) and anemia (hemoglobin 8.9 g/dl). The other tests were normal. The patient underwent mastectomy and the pathological result showed metastatic germ cell tumor including yolk sac tumor and embryonic carcinoma and few syncytiotrophoblasts. Chemotherapy with BEP protocol was initiated for him but the patient was expired. Conclusion: Regarding the presented case, assessment of breast bulging and pain in cases with a history of testicular germ cell tumors, should attain more precaution to rule out the metastatic cases because prompt diagnosis and treatment would be life-saving.
