Surgical outcome of retinotomy and retinectomy in patients with proliferative vitreoretinopathy : 8years experience from tertiary hospital

Purpose: To report the anatomic and functional outcomes of retinotomy and retinectomy for the management of retinal detachment complicated by proliferative vitreoretinopathy (PVR). Methods: In this retrospective study, the charts of patients who underwent pars plana vitrectomy with retinotomy and retinectomy for the management of retinal detachment complicated by PVR from 2010 to 2018 were reviewed. Primary outcome measures were final visual acuity and anatomic success rate. The cases with at least 3 months follow up post-operation and complete file were included. Results: Finally, 70 eyes of 66 patients were enrolled. Of them 62.9 % were male. The mean age was 43.49 19.81. The mean follow-up time was 25.66 25.14 months. The lens status was aphakia in 28.6%, phakic in 17.1% and pseudophakia in 54.3% of the patients. The Best visual acuity was 2.64 0.88 and 2.30 1.06 LogMAR before the surgery and at last visit (P=0.015). The retina was attached after one surgery in 90% of thepatients. Twenty-two eyes (31.4%) need reoperation with the mean time of 5.76 5.29 months after the 1st surgery. Finally, the retina was attached in 92.7% of the patients at last visit. The silicone oil tamponade was present in 71.4% of the patients at last visit. The mean size of the retinotomy was 158.09 93.23. Spontaneous retinectomy was performed in 20% of the cases. Conclusion: Retinotomy and retinectomy is a surgical option providing good anatomical and reasonable visual outcomes in complicated retinal detachment with PVR.
