Evaluation of correlation between pathologic results of transthoracic needle biopsy and lung high resolution CT scan findings in lung lesions

Introduction: Trans thoracic needle biopsy (TTNB) is a rapid and minimally invasive method of biopsy in intraparanchymal lung lesion particularly peripheral lesions that are not available for bronchoscopy. The aim of this study was evaluating the correlation between TTNB results and lung HRCT finding in lung lesion. Materials and Methods: This retrospective study was performed in Lung Disease Research Center, Mashhad, from 2007 to 2017. At first, all patients who had TTNB were selected.After reviewing the charts, 110 patients were enrolled in the study. Demographic, radiologic, and pathologic data were recorded. The correlation betweenpathologic and radiologic findings were evaluated. Results: The median age of patients was 60 (26-66) years. 64% of patients were male and 36% were female. The most frequent finding of TTNB in both males and females were malignancy (46%in male and 42% in female). Additionally, malignancy was the most frequent diagnosis in patients older than 50 years. Kapa coefficient for determining the correlation was 0.498 in ILD (p value),0.30 in malignancy (p value)and 0.075 in TB(P value). The most sensitivity of HRCT was in malignant lesion (100%) and the most specify was in Interstitial lung disease (96.5%). Conclusion: In summary, this research show that accuracy of lung HRCT for diagnosis of lung lesion is different between types of lung lesions. But in many cases, for definitive diagnosis, we need biopsy and histopathologic examination.
