Over the past decade, percutaneous RF ablation is introduced as an important therapeutic option for VT in patients with and without structural heart disease. In patients without structural heart disease, RF ablation serves as the therapy of choice for the treatment of VT. For patients with structural heart disease, VT ablation has usually been reserved for patients who experience ICD shocks and particularly those who fail antiarrhythmic agents. Recent advances have enabled noninvasive mapping of cardiac arrhythmias with electrocardiographic imaging and noninvasive delivery of precise ablative radiation with stereotactic body radiation therapy (SBRT). SBRT simulation, planning, and treatments will be performed with the use of standard techniques. Patients will be treated with a single fraction of 25 Gy while awake. Efficacy was assessed by counting episodes of ventricular tachycardia, as recorded by ICDs analysis. In all patients with refractory VT, noninvasive treatment with electrophysiology-guided cardiac Radio-Ablation markedly reduced the burden of ventricular tachycardia.
