چکیده مقاله:

Introduction:Tracheal stenosis can be a life threatening disorder and intratracheal stenting in pediatric patients with tracheal stenosis is an alternative method when surgical correction cannot be performed. The case: Our patient was a one year old boy 8 Kg that after open heart surgery developed long segment tracheal stenosis from underneath of the vocal cords to the carina, following long term tracheal intubation and intensive care support.Meanwhile we were not able to extubate him, he began more respiratory difficulty and ventilator-associated pneumonia. After consultation with surgeons and other colleagues we decided to perform an intratracheal stenting by means of a bronchoscopic method. Therefore a bare GENESIS XD peripheral stent 36 mm, that was the best available stent mounted on a 12*40 mm CRISTAL Balt balloon and inflated on 0.035 guide wire and after that he was referred to the intensive care unit with spontaneous respiration and fortunately discharged from hospital one week later in a fair condition. After about one month we asked our pediatric surgeon to remove the stent in the operation room. Rigid bronchoscopy showed thick granulation tissue around the zigs and after electrocauterization of them our colleague could remove it without bleeding or complication.Several bronchoscopies up to about 10 months showed normal tracheal diameter with no stenosis.Acknowledgement: We thank our pediatric pulmonologist for the outstanding role in this work.
