Comparative efficacy of intralesional cryotherapy vs intralesional Corticosteroid and 5 Fluorouracil in treatment of hypertrophic scars and keloids

Background: Keloids are fibrous scars that go beyond the margins of the primary ulcer and usually recur after surgical removal. Hypertrophic scars are similar lesions, but they were limited to scar edges and regression were seen overtime. Treatment of Scars is an old challage and single effective treatment has not yet been proven for it. The aim of current study was to compare intralesional cryotherapy with two different triamcinolone and 5 fluorouracil intralesional injection. Materials & Methods: The study was done in Sina hospital during May 2016 to August 2017 in patients with keloids or hypertrophic scars. Totally 21 scars was distributed in each group, in first group intralesional cryotherapy, 2nd and 3rd group was treated intralesionally by triamcinolone 0.2 ml/cm2 and 5florouracil 0.2ml/cm2. The dimensions of the scar (include: surface, height and volume) were measured before and after the start of the study. Treatment was repeated every 4 weeks, depending on the presence of scar tissue, up to 6 sessions. Results: Significant decrease of the surface, height and volume of keloids was seen after first session in comparison to other methods. In steroid, decrease of surface, height, and volume after 6 sessions was 52.9, 61.37, 78.06 percent and in 5FU groups was 32.16, 58.07, 60.67 percent, respectively. The results indicated there was significant difference between steroids and 5FU groups in point of surface, height, and volume, and the results of better in steroids group after six sessions. The complications in steroids group was permanent hypopigmentation, telangiectasia, atrophy and in 5FU group was include surface wound, hyperpigmentation, pain increase but in cryotherapy only temporary hypopigmentation was reported. Conclusion: The results indicated that the cryotherapy promotes quick keloids recovery and its complications was lower than other methods.
