Evaluation of the relationship between postnatal hypoglycemia and the result of umbilical artery Doppler ultrasonography in neonates with intrauterine growth restriction in third trimester of pregnanc

Background and Aim : Intrauterine growth restriction (IUGR) refers to fetuses, which their estimatedultrasonography weight is less than 10% percentile. Hypoglycemia is a major concern in neonates withgrowth restriction. Umbilical artery Doppler shows resistance to embryo-fetus blood perfusion, which leadsto multiple complications. As embryo-fetus blood perfusion conditions can be monitored in IUGR neonateswith umbilical artery Doppler, this research was conducted to evaluate the relationship between umbilicalartery Doppler and neonate hypoglycemia and growth restriction.Evaluation of the relationship betweenpostnatal hypoglycemia and the result of umbilical artery Doppler ultrasonography in neonates withintrauterine growth restrictions in the third trimester of pregnancy.Methods : The study was conducted on 114 neonates with intrauterine growth restriction, with gestationalage of 28-40 weeks from April 2016 to October 2017 at Shahid Sadoughi Hospital, Yazd. In terms of typeof Doppler ultrasonography, they were assigned into three subgroups of normal Doppler, absent Doppler,and reverse Doppler. Blood glucose of neonates was checked during the first hour of birth. Then, bloodglucose was taken 2, 3, 6, 12, 24, and 48, hours after birth and the neonates were placed in hypoglycemiagroup or euglycemia group according to the guideline and based on the number obtainedResults : Out of 114 samples included into the study, 75 (65.8%) had normal umbilical artery Dopplerultrasonography, 29 (25.4%) had absent umbilical artery Doppler ultrasonography (lack of diastole), and10 had reverse umbilical artery Doppler ultrasonography. The data were analyzed by using SPSS 18. Giventhe ANOVA and chi-square tests and P value = 0.001, there was a significant difference in mean of bloodglucose in the first hour in 3 groups of Doppler ultrasonography and the difference was seen between twogroups of normal umbilical artery Doppler ultrasonography and reverse umbilical artery Dopplerultrasonography (P value = 0/002).Conclusion : According to the findings of this study, it can be concluded that postnatal hypoglycemia inIUGR neonates is associated with result of umbilical artery Doppler ultrasonography during pregnancy, sothat reverse umbilical artery Doppler ultrasonography significantly increases the probability ofhypoglycemia in the first hour of birth.
