Uterine Intracavitary Angioleiomyoma Treated by Minimally Invasive Method: A Case report

Background and Aim : Angioleiomyoma is an infrequent type pf leiomyoma involving the female genitaltract with similar clinical manifestations including abdominal pain, bleeding or pressure effect.Methods : The patient underwent hysteroscopic evaluation. Under diagnostic hysteroscopy with 4.5 mmhysteroscope and 11 mm resectoscope (Karl Storz SE & Co. KG, Tuttlingen, Germany)Results : In this study we describe a case of angioleiomyoma in a 41-year-old woman with abnormal uterinebleeding following an induced abortion. Transvaginal ultrasound showed a heterogeneous echogenic lesionin the uterine cavity which was highly suggestive of retained placental tissue. Under resectoscopy, aftershaving of more than 60% of mass, visualization was limited due to excessive bleeding. The resectoscopewas removed, and the remaining mass was grasped by ring forceps, twisted and extracted. A three-waysilicone Foley catheter (size 16 French) was inserted into the uterine cavity and the balloon filled with 6mL sterile water. Intravenous antibiotics were started and after twelve hours the balloon was graduallydeflated and removed on the same day. The follow-up ultrasound images of the uterus at three, six and ninemonths were normal.Conclusion : Uterine intracavitary angioleiomyoma is uncommon, hysteroscopic view and clinicalmanifestations are similar to uterine leiomyoma. Despite the hypervascular nature of these lesions andexcessive bleeding during hysteroscopic resection, the surgeon can still preserve uterus by minimallyinvasive methods.
