Background and Aim : Sedation is one of the most important measures to help co-operate ofyounger patients. The aim of this study was comparison of efficacy of oral midazolam andketamine sedation in children referred to the emergency department.Methods : In this double-blind clinical trial, the children were in grade one or two based on ASAcriterion, was selected. Then they randomly divided into two groups of 50. To first groupMidazolam administered orally (0.5 mg/kg) and second group Ketamine administered orally (5mg/kg).Results : The mean heart rate of patients before intervention was not significantly differed in thetwo groups, and this difference was not statistically significant. Meanwhile the mean heart rate ofpatients after intervention was statistically significant. (p=0.018) Average scores for sedation andseparation of children from parents between two groups (Ketamine and Midazolam) before andafter intervention was not statistically significant at 10, 20, and 30 min after administration ofdrugs. Average of time of stay in hospital after sedation in two groups was statistically significa nt,in Ketamine group was 28.62 min and in Midazolam group was 34.04 min(p=0.007).Conclusion : In Ketamine group, average of heart rate had less reduction after intervention incomparison to Midazolam. Time of stay in hospital in ketamine group was shorten in comparisonto Midazolam. Two criterion that affect satisfaction of patients and their parents. It is notable thatpercent of patients with the start of sedation at 20 min after ingestion of drug, was higher inKetamine group in comparison of Midazolam group. It seems that Ketamine had greater effect forsedation of children need radiography or urgent procedures, than Midazolam
