Safety indicators: urgent need of our oil and gas industries.

Despite of huge number of publications and methods of hazard identifications, accidents occasionally occur. Texas City refinery explosion is an manifested case of discrepancy between our traditional accident and facts in our industries. The proper way in these situations is that we discriminate with personal and process safety indicators. And we keep in mind that each type of these indicators could occur in tow mode of leading and lagging. Unfortunately in our oil and gas industries we only record and monitor lagging personal indicators. And overlook three other types of indicators. Finally we conclude that our oil and gas industries must paid more attention to all four block of safety indicators to prevent process and personal accidents.
