Second primary tumors in patients with breast cancer : The incidence and risk factors

چکیده مقاله:
Breast cancer is the first most common cancer in Iranian women. Improvement in cancer treatment have increased the chances of survival, results in greater number of second primary cancers ( SPC) . The aim of this study is to determine the incidence and types of SPC as wall as possible risk factors in patients treated for breast cancer .Methods and Materials :All patients with primay breast carcinoma from 1995 to 2013 , referred to Omid and Reza oncology hospitals (Mashhad,Iran) ,entered to this retrospective study. Patient,s age , initial histology ,type of surgery and history of radiotherapy (RT) among those with SPC were extracted from the medical records. Any relation between these features and SPC were analysed using binominal test.Results:After median follow up of 60 months (range 10-324), SPC was documented in 34 (1.2%) of 2787 breast cancer survivors.Contralateral breast carcinoma (CBC) was developed in 23 (67%) cases.Other second malignancies were in endometrium (2 , 6%),ovary (3 , 9%),brain (2 , 6%),esophagus(1 , 3%),bone marrow (1 , 3%),connective tissue (1 , 3%) and skin(1 , 3%).The median age at diagnosis for CRC was 48 years (range 27-76). Invasive ductal,lobular and medullary carcinoma were reported in 10 (43%),3 (14%) and 10 (43%) patients respectively.The majority of CBC cases(19, 83%) underwent mastectomy.17 patients in RT group versus 5 patients in no RT group developed CBC which was statistically significant (p=0.017).There was no significant relation between patient,s age , initial histology ,type of surgery and CBC.Conclusion:There is a known risk of CBC, especialy in those have received RT.This increase should be in mind during clinical decision –making.
