Introduction & Aim: Breast cancer is the most frequently diagnosed malignancy for women worldwide. Alterations in the One-carbon and folate metabolism pathway can influence DNA methylation status, DNA synthesis and repair which play a critical role in carcinogenesis. Methionine synthase (MS) gene encodes a vitamin B12- dependent enzyme that catalyzes the remethylation of homocysteine to methionine. The common polymorphism of MS is A2756G that induces modest homocysteine reduction and DNAhypomethylation. The purpose of the present study was to investigate the frequency of folate-related polymorphism (MS A2756G) and its possible association with susceptibility to breast cancer in Kermanshah Province Methods: A total of 249 subjects (98 breast cancer women and 151 healthy controls) were genotype forMSA2756G polymorphism was analyzed by PCR-RFLP method. Results: The allelic frequency of MS 2756G allele was 21.9% in patients compared to 28.5% in healthy subjects that did not reach to a statistical significance (P = 0.1). Conclusion: Our study has demonstrated the frequency of one of genes involved in the folate metabolism in a homogenous ethnic group of breast cancer patients. Our data indicate that MS A2756G polymorphism might not be a risk factor for susceptibility to breast cancer in Western Iran
