A Comparative Study: Impact of Early Maladaptive Schemas on treatment Outcomes for Cognitive Behavioral Therapy and Systematic Desensitization in Obsessive Compulsive Disorder

The importance of early development in the etiology of OCD has long been established, however, the role of resulting cognitive schemas on treatment outcomes are only recently being considered. The current study evaluates the role of Early Maladaptive Schemas (EMS) in predicting reduction of obsessive-compulsive symptoms after treatment using Cognitive Behavioral Therapy (CBT) and Systematic Desensitization (SD). A total of 38 participants were randomly assigned to two groups, where the first group received SD and the latter one CBT. The Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) and Young Schema Questionnaire Short Form (YSQ-SF) were administered before and after treatment. Results of a mixed model repeated measures ANOVA revealed that individuals in the CBT group showed significantly greater improvement in total OCD symptoms compared to those in the SD group. In the SD group, individuals at low EMS levels benefitted most from this treatment, compared to those at medium and high EMS levels. In the CBT group, individuals at low and medium EMS levels benefitted significantly more than those at high EMS levels. Breaking symptom scores down to their Obsession and Compulsion subscales revealed that only individuals in the CBT group experienced a significant reduction in obsession symptoms, with those at medium EMS levels experiencing a greater reduction in obsessive symptoms compared to those at low and high EMS levels. Both treatments were effective in reducing compulsion symptoms, however, CBT was overall more effective than SD at low and medium EMS levels. There was no significant reduction in symptoms in either the CBT or SD groups for individuals at high EMS levels. Results demonstrate a systematic difference in therapeutic outcomes of SD and CBT as a function of the underlying severity of early maladaptive cognitive schemas. While findings further support the importance of cognitive treatment components offered by CBT in reducing overall OCD symptoms, especially for those at moderate EMS levels, it highlights the importance of improved treatment approaches for OCD in individuals with high EMS levels.
