Background and Aim: Cancer is the second leading cause of death globally. Iran has the highest mortality rate of gastric cancer in South-Central and Western Asia. Among a number of hypotheses, compounds with antioxidant properties have been proposed to prevent gastric cancer. Selenium is an essential mineral having considerable role in anti-oxidation and anti-inflammatory pathways. We conducted a case-control study in the Cancer Institute of Iran to evaluate the association between selenium intake and gastric cancer.Methods: Cases were 178 histopathologically confirmed gastric cancer patients and 276 controls were selected from healthy persons coming to visit their relatives. Controls were frequently matched to cases by age and sex. Trained dietitions conducted face to face interview to complete the validated version of Diet History Questionnaire. It included 146 questions related to past 12 month consumption of foods. Iranian and McCance and Widdowson s food composition tables were used to estimate selenium content of foods. Odds Ratios (OR) and 95% confidence intervals of gastric cancer were calculated using unconditional multiple logistic regression models. Results: The mean of selenium intake was 87±43.27 and 89.4±54.78 microgram per day in case and control group respectively. We found that selenium intake was inversely associated with gastric cancer (OR for the second tertile versus first= 0.51, 95%CI= 0.31-0.85). The association was significant after adjusting for education and residence (OR = 0.47, 95%CI=0.27-0.82, P for trend=0.23).Conclusion: Selenium intake may protect against gastric cancer. Cohort studies are needed to verify it.
