ASSOCIATION OF BMI WITH WOUND HEALING AND CLINICAL OUTCOMES IN HOSPITALIZED MODERATE BURN PATIENTS

Background and Aim: Obesity represents a global epidemic and is recognized as a major contributor to the burden of chronic disease and disability worldwide. Body mass index (BMI), embraced by the World Health Organization in 1995, is the most frequently used tool to categorize obesity. Obesity is an important predictor of mortality and morbidity during a hospital stay. There is very little data concerning the impact of the BMI on clinical outcomes in obese burn patients The purpose of this study was to determine the effect of obesity on clinical outcomes after burn injury.Methods: 130 patients who were hospitalized in the burn unit were enrolled in this prospective study. Serum levels of albumin, height and weight were measured for each patient within the first 24 hours of admission. Patients based on BMI were divided into four groups: underweight (<18.5), normal (18.5-24.9), over weight (25-29.9), and obese (> 30). Patients were followed for length of hospital stay, wound healing, infection and mortality. Results: Overweight and obese Patients had significantly longer length of hospital stay (p=0/03 in overweight and p<0.001 in obese). Overweight and obese Patients had significantly longer duration of wound healing but in multivariate logistic regression model it was not significant. Overweight, obese and underweight Patients had significantly more infection than normal group (p=0.03 in underweight and p=0.001 in overweight and obese). Conclusion: Obesity is an independent predictor of adverse events after burn injury; our findings suggest BMI as a risk factor for longer length of hospital stay and more infection in overweight and obese patients.
