Infants who were born preterm and require neonatal intensive care remain at increased risk for morbidity and mortality following discharge from the neonatal intensive care unit (NICU.( Careful discharge planning is required to minimize the risk of morbidity and mortality from premature discharge and to prevent prolongation of the hospital stay in an infant ready for discharge. It includes assessment of the neonates medical status and readiness for discharge and preparation for families to care for their infant at home. Discharge planning should be developed by a multidisciplinary team consisting of parents, neonatologists, nurses, physical therapists, dieticians and pharmacists. Discharge planning consists of three parts: o Neonatal planning O Family planning 0 Transition planning and post-discharge care In neonatal planning we must check neonatal status and readiness for discharge. The neonate must be able to maintain temperature in open crib and adequate oral feeding to promote appropriate growth and weight gain. He must be having adequate cardiorespiratory control without episodes of apnea and bradycardia at least five days after discontinuation of caffeine before discharge. We should complete the routine screening including: Metabolic and genetic disorders (if needed Retinopathy of prematurity for infants with gestational age <30 weeks and birth weight<1500 gm Hearing screening using auditory brainstem response Other screening tests based on clinical settings may include brain imaging and laboratory studies (eg, complete blood count or alkaline phosphatase e. Routine immunization based on chronological age In family planning we must consider Parental education and readiness and competency in the daily care of their infant and demonstrate knowledge of normal behavior of their infant and ability to recognize signs and symptoms of illnesses that require medical consultation In Transition planning and post-discharge care Specialty follow-up and If appropriate, appointments scheduled for post-discharge follow-up with subspecialty services must be considered.
