Neurocritical care is one new strategy to improve the neurological outcome of the newborns with acute neurological problems in the neonatal intensive care unit(NICU). Neonatal Neurocritical care (NNCC)is an evolving subspecialty in neonatal medicine. The NNCC use neuroprotective strategies for lessening the risk of neurological morbidity in critically ill neonates. Because the neonatal brain is still developing and vulnerable to injury, the essential keys of NNCC strategies are identifying the time of brain injury in at-risk infants.NNCC is one multidisciplinary care by a neonatologist, pediatric neurologist, NICU nurse, physical and occupational therapist, and psychologist. Four main components of NNCC includes assessment, monitoring, protection, and development. Neurological exams such as Sarnat exam (encephalopathy staging), pain scoring, and sedation indexes introduced for neonatal assessment in NNCC. The aEEG(amplitude-integrated EEG), cEEG(continuous Electroencephalography), NIRS(Near-infrared spectroscopy) are some of the monitoring technologies are used. Significance of the role of NICU nurse in neuroprotective support of newborn demonstrated in multiple studies. Neurodevelopmental status improves in NICU with reducing of intraventricular hemorrhage, decreasing pain sensation or stress tolerance in the preterm infant withsimple acts such as the implement of positive touch and the presence and interaction with parents in the NICU. The means of developmental strategy in NICU is an environmental improvement, the lessening of light and sound stimuli, the reduction of sedation for patients and improves the new technology such as hypothermia.Education of the neonatal parents in NICU to learning how they should take care of them has a lot of value for their short and long term neurological prognosis. Sometimes, psychiatric consult for parents to alleviate anxiety, depression, and posttraumatic stress disorder in their family is helpful. Evidence-based studies display the whole of these proceeding may result in decrease the neonatal neurodevelopmental disorders in NICU.So hopefully, they also can help improve the quality of neonatal care in developing countries.
