Introduction: Polycystic ovarian syndrome (PCOS) or hyper-androgenic chronic anovulation is the most common female endocrine disorder that affects more than 5% of women in the reproductive age. PCOS has a wide variety of clinical manifestations: from premature adrenarche signs to alterations of the menstrual cycle secondary to ovulatory dysfunction, to dermatological manifestations, such acne and hirsutism, to metabolic alterations, to cardiac and psychological problems and cancer all along the life period often accompanied by ultrasound features of polycystic ovaries.The ultrasonographic diagnosis of PCOS is reported to be still an open problem. The frequency of over diagnosis in particular phase of reproductive age and missed diagnosis, the contrasting guidelines and the technological advances lead to be a difficult charge for the sonographer. Every PCOS case is unique.Methods: All the factual and evidential information items are demonstrated in a single power point slide to provide the conclusive aid to precise medical practice and workup of PCOS cases in the context of the Iranian traditional medicine. The animation of this power point presentation is precisely designed as to provide an accurate understanding for the undergraduate medical students about the subject. A PCOS case is described as an example after a full work-up over 9 years of follow up. The precision of Vitagnus treatment pharmacogenomics is discussed over and cross validated on the model offered.Results: Gonad temper as a fifth level of basic temperament, proper sequential order of tempers, internal and external interactions accounting for temper variability, exposomic and dynamic of overall temper equilibrium are systematically put together to offer basics of a helpful lifestyle and longtime consideration for the management and health state maintenance in PCOS cases.Conclusion: A clear understanding of the temperaments and other basic concepts in Iranian traditional medicine and a precise case study analysis can provide rather precise and promising clinical recommendations in the optimal management of a PCOS case.
