Comparison of hematologic indexes in Pvera and secondary polycythemia

Introduction: Polycythemia is divided into three types: Polycythemia Vera, secondarypolycythemia mainly due to the increase in erythropoietin in patients with low oxygen level causedby pulmonary disease or erythropoietin-producing tumor; and relative polycythemia caused byobesity and anxiety. The first two types and the last type show an absolute and relative increase inthe number of RBC, respectivelyMethod: We evalute 58 patients who referred to hematology clinic in shiraz. Complete bloodcount, and carful physical examination for sing and symptoms of polycythemia includingsplenomegaly, hypertension and history of recent or previous thrombosis were evaluated. JAK2mutation for confirmation of P.vera was done. The type of smoking was delineate.Results: Total 58 patients entered in this study .37 males and 21 females, 33 patients hadPolycythemia Vera,20 patients had secondary type and with relative polycythemia. Mean age was52.22±15.94(17-75Y), mean Hb was18.26±2.30, mean Hct was 55.18±6.25. The difference in meanof WBC count, MCH,MCHC,Platelet count were statistically significant.(p=0.002,0.006,0.001,00005). These are means that in P.vera mean WBC count ,MCH and plateletcount was higher, but the Hb MCHC significantly higher in secondary polycythemia Patientswith secondary polycythemia have lower platelet count especially in patient that use Hubblebubble.in cigarette smokers mean Hb was 18.8g/dl (17.1-24.9) and mean platelet count was288280(101000-502000) and in Hubble bubble smokers mean Hb and platelet counts were19.65g/dl (17.9-23.7) and 215930(72000-754000) respectively. Hypertension was more prevalent inP.vera group (24.24%) and 20% in secondary polycythemia.JAK2 mutation was positive in 60.5% only in P.vera group. Also thrombosis was more prevalent in P.vera in comparison to secondarypolycythemia 15.15% and 15% respectively.Conclusion: Patients with secondary type of polycythemia have higher Hb and Hct level and lowerWBC and platelet count. Significant mild thrombocytopenia below 150,000 was seen in patientsthat use Hubble bubble. Hypertension and thrombosis were more significant in Polycythemia Veragroup
