Alveolar ridge preservation after tooth extraction using PRF versus bone allograft: A clinical study

IntroductionBone remodeling after tooth extraction often results in decreased ridge volume, and complicates implant placement. Platelet rich fibrin is a rich source of autogenous cytokines and growth factors that can effectively improve soft tissue healing, hard tissue regeneration, and ridge preservation.ObjectivesA randomized, prospective clinical study was designed to evaluate bone healing after alveolar ridge preservation technique using two different graft materials, Platelet Rich Fibrin as the sole grafting material and Freeze-dried bone allografts (FDBAs).MethodsThis clinical trial was conducted on 32 patients presenting to Tehran School of Dentistry for extraction of hopeless nonmolar teeth. The teeth were extracted atraumatically and randomly divided into two groups (n=16). Tooth socket was filled with FDBA in group one and PRF (prepared using 10cc of patient’s blood) in group two. For assessment of bone regeneration, changes in horizontal and vertical bone dimensions were assessed at the extraction time and 12 weeks later (at the time of implant placement) using a periodontal probe and an acrylic stent fabricated before tooth extraction. The results were compared between the two groups.ResultsImplants were inserted after 3 months in all cases. Ridge width showed a significant reduction in the two groups after treatment compared to the baseline values (P=0.001). Changes in ridge height were not significant after treatment in two groups compared to baseline (P> 0.05). The changes in ridge height or width were not significantly different between the two groups (P> 0.05).ConclusionThe results showed optimal efficacy of PRF as the sole grafting material for alveolar ridge preservation after tooth extraction for implant placement. It is affordable and easily prepared, and its application in the extraction socket yielded results similar to those of FDBA.
