The Effect of the Doula support on Episiotomy Rate In Primipara Women

Introduction: Routin episiotomy have several benefits for mother and neonate such as prevention of perineal damage ,urinary incontinence,intracranial hemorrhage but studies show that episiotomy may have complications like bleeding,infection, rupture grade 3 and 4 perineum.Since the presence of a trained attendant midwife (doula) beside the partituate may facilitate the uterus blood flow and contractile activity through reducing need eoisiotomy, this study was performed with aim to determinethe effect of continuous care by trained doula on episiotomy rate.Material&Methods: This clinical randomized trial was performed on 106primigravida women referred to Shafa Hospital of Semnan for delivery. women received in addition to routine care of delivery room were supported by a trained midwife. Data collection tools included the questionnaire of demographic characteristics and check list for determine the need for episiotomy and complication . Data was analyzed by SPSS(16 v).Results: Results showed that significant difference between two groups,so that in intervention group the amount of episiotomy needed to be reduced (P<0.001). Also infection rate in group with doula, significantly decreased but bleeding rate was no difference beetwin two groups.Conclusion: The support of a trained attendant midwife doula beside the partituate reduces the number of women who need episiotomy and this has a positive effect on mother and baby healh .
