Back ground: Mortality rate is different in studies evaluating patients with multiple sclerosis (MS). As the MS related hazard ratio (HR) varies in different studies, we aimed to conduct this systematic review and meta-analysis to estimate a pooled hazard ratio (HR) of MS-related death .Methods: We searched the databases including PubMed, Scopus, EMBASE, Web of Science, , google scholar and gray literature such as references of references, congress abstracts. The search included articles which were published up to the first of May 2019.Results:The first search revealed 7080articles while after deleting the duplicates 2504remained. After deletion of irrelevant studies, 156 and finally 39 found for full text evaluation. At the end, 5 articles included in the meta-analysis.Two studies were from United Kingdom, one from Canada, one from Italy and one from Sweden. HRs were ranged from 1.64-3.5, with different adjusted items. Pooled HR estimated as 2.35 (95% CI:2.02-2.74) (I2=90.7%, P<0.001).Conclusion: The result of this systematic review showed that the pooled hazard ratio of mortality in MS cases is 2.35 fold comparing with controls.
