Background and Objective: geographical differences in Immunization programs are a major public health problem, which is often linked to the availability of healthcare services and socioeconomic factors. The aim of this study was to evaluation of the coverage of Immunization and sociodemographic Factors Affecting Immunization Status in children under one year of urban and rural areas of Zahedan city.Materials and Methods: The present study is a cross-sectional study that was conducted on children under one year old urban and rural areas of Zahedan city. The sampling method in this study was probabilistic proportional to size sampling. 400 children in urban areas and 400 children in rural areas were examined by questionnaire. data were analyzed using descriptive statistics and chi-square and Fisher s exact tests.Findings: 92.4% of boys and 90.8% of girls,were fully immunized during the study period. In urban areas, the highest percentage of immunization coverage was reported for BCG, HB, OPV0, Pentavalent vaccines( 100%). The lowest percentage of immunization coverage in urban areas was the IPV vaccine (94.5%) and in rural areas coverage of OPV0 ,HB had the highest percentage (100%). The lowest percentage of immunization coverage in rural areas was related to the vaccine of IPV(90.8). The pattern of missed vaccines in children living in rural areas for multiple doses vaccine was significant differences than Children living in urban areas .Conclusions: Efforts to improve vaccination coverage children should be taken into account regarding in the rural and urban areas. monitoring and control of vaccination to improve the status, as well as training classes for mothers about the immunization program is essential
