The efficacy of Modification of Diet in Renal Disease formula in estimating glomerular filtration rate in patients with normal serum creatinine

Background and objective: Chronic kidney disease (CKD) is the impairment of kidneys. Glomerular filtration rate (GFR) is an increasingly important factor in diagnosing CKD. Modification of diet in renal disease(MDRD) formula is a tool for estimating GFR which is related to some factors like age, weight and serum creatinine. In this study, we aimed to investigate the efficacy of MDRD equation as a diagnostic factor in patients with normal serum creatinine. Materials and Methods: In this cross-sectional study, by convenience sampling, 200 outpatients between 20 to 60 years old which had normal serum creatinine measurements who referred to 22 Bahman Hospital of Mashhad, from October 2007 to January 2009 were enrolled. Pregnant women, paraplegic and quadriplegic patients, participants with BMI> 30 and BMI<15, diabetes (FBS> 126), high blood pressure (pressure> 140/90), and patients with a history of Cimetidine, Trimethoprim, Cephalosporin and Probenecid use were excluded. Based on the classification of CKD, we calculated the frequency distribution of GFR according to three factors: gender, age and creatinine. Also we calculated GFR with MDRD equation for patients. Patients were classified to four stages; stage 0 (GFR> 90 ml/min), stage 1 (GFR= 90 ml/min), stage 2 (60< GFR<90 ml/min) and stage 3 (GFR ≥60 ml/min). Outcomes were analyzed with SPSS version 11 and Fischer test was used. All patients filled consent forms and the research ethics committee approved the study Findings: Out of 200 patients,42.5% were male and 57.5% were female. The average age of patients was 39.4±11. The mean weight of patients was 73.3±14.6. There was a significant association between gender and high stages of CKD (P<0.0001). There was also a significant association between stage of CKD and increasing in age (P=0.005). We observed that increased level of creatinine had a significant association with higher stages of CKD (P<0.001). After GFR estimation, participants were classified with MDRD formula according to CKD stages. 168 patients were classified as stage 0 of CKD, zero as stage 1, 29 as stage 2 and 3 patients as stage 3. Conclusion: This study has shown that the MDRD formula is a confidential method for evaluating GFR in patients with normal serum creatinine.
