Human T cell lymphotropic virus-1 (HTLV-1) is an oncogenic retrovirus which can cause adult T-cell leukemia lymphoma (ATL). Infection with this virus is endemic in some parts of the world includingKhorasan Razavi in Iran. ATL is an aggressive T-cell proliferation with a poor prognosis. ATL symptoms include organomegaly, cutaneous lesions and hypercalcemia.The proviral load (PVL) is defined as the percentage of HTLV-1–infected peripheral blood mononuclear cells (PBMCs). We reviewed the literature to present the applications of the PVL in ATL. The databases of PubMed, Scopus, ISI Web of SCIENCE, MEDLIB and SID were searched using HTLV-1, ATL and proviral load.It is shown that the PVL usually remains relatively constant over years within an individual and rises slowly over decades. The most important predictor of clinical disease in ATL is the proviral load. It has been suggested that those patients who have a high proviral load level (more than 4 copies/100 PBMCs) are in a high-risk group for developing ATL.The proviral load level of asymptomatic carriers who had a family history of ATL was significantly higher than for those with no family history.Also the median proviral load of HTLV-1 carriers with comorbidities such as infectious and autoimmune disease was significantly higher than that of those with no comorbidity.ATL is divided into four clinical subtypes which are acute, lymphoma, chronic and smoldering types. The HTLV-1 proviral load hasbeen shown to be significantly increased in the acute and chronic type. Therefore determining the HTLV-1 proviral load is useful for distinguishing among the different types of ATL. Measurement of proviral load by TAX expression is a valuable tool for monitoring viral activity in ATL patients who are under treatment. It is necessary to determine the PVL in all HTLV-1 infected subjects in order to predict the development of ATL
