Development and implementation of burned hand care protocol

Background: Evidence-based nursing protocols, based on the latest studies, are helpful for decision-making in clinical practice. The purpose of this study was to develop evidence-based protocol for burned hand care.Method: This systematic review is part of a research project approved by Tabriz University of Medical Sciences after obtaining an ethical code (IR.TBZMED.REC.1396.975). All studies related to hand burn injuries from PubMed, Medline, clinical key, Elsevier, Google scholar database from 2000 to 2018 were gathered. From 2382 articles, 43 of them were selected according to inclusion criteria. Related studies were assessed in two stages by research team. Firstly, titles and then abstracts were reviewed. Ultimately 20 article were included for developing hand care protocol.Result: Selected studies consisted of 15 RCTs and 5 hand care Guidelines. Hand burn care protocol’s draft was written based on these studies. Completed protocol was developed after the approval of a panel of experts. The protocol consist of hand burn wound care at different phases of treatment regarding initial and daily assessment of wound, dressing, hand exercises, polyurethane gloves, nutrition. Also, Patient training was developed based on the nursing process, which presented at original article.Conclusion: This protocol was run as a pilot for 6 months in Tabriz Sina Hospital at burn wards. Along with other studies, it shows that evidence-based protocols improve the quality of nursing care and reduces the distance between theory and practice. The current protocol reduces personalization care and allows burn wards nurses to do alternative procedures step by step to solve problems.
