Introduction: Squamous cell carcinoma (SCC) is a malignant tumor of epithelium with a high mortality rate second to malignant melanoma. It isthe second most common cancer of the skin but scc of eyelid is relatively uncommon, specially of upper lid. we describe an old woman with chronicocular discharge that confirmed as a subtarsal Squamous cell carcinoma (SCC).Case Presentation: An 81-year-old woman complained ocular chronic discharge that was present for 1years was interfering with his vision and thathis family was embarrassed by his appearance. she had multiple treatment as a infective or allergic conjunctivitis without improvement for several months. She had progressively massive serosanginous discharge . In careful examination After lid eversion There was a multinodular mass with several granulation tissue arising from upper border of subtarsal conjunctiva of upper eyelid . she had severe discomfort and recurrent bleeding andserosanginous discharge and was systematically well. she reported progressive increase in discharge . she had severe right mechanical andinflammatory ptosis with compensatory bilateral frontalis muscle overaction. His visual acuity was 20/60 OU (when the eyelid was lifted offthe visual axis), and ocular findings were normal. He underwent an excisional biopsy under local anesthesia, which included part of theunderlying central part of levator aponeurosis tendon and most of the tarsal plate resected as high as accessible and visible but spared the lid margine and eyelashes. Clinical suspicion was first pyogenic granuloma or embedded foreign body then sebaceous cell carcinoma . pathological report confirmed squamous cell carcinoma that has not been reported in this site yet. We treated patient with radiotherapy for residual tumor cells. She is stable without reccurence in 6 months.Conclusion : Eyelid SCC is a relatively uncommon, but potentially fatal disease. Subtarsal area is very rare area for scc . the most risk factor is UVexposure and so subtarsal area without exposure SCC is very uncommon . we report first case of subtarsal scc with successful treatment with surgical excision and radiotherapy
