Atypical fibroxanthoma of eyelid with canalicular involvement initially presented as a chalazion: A case report

Introduction: Atypical fibroxanthoma (AFX) is an uncommon pleomorphic cutaneous tumor mostly appears on the sun damaged skin of elderly patients that rarely located in the periocular region. It rarely occurs on the eyelid. We report a rare case of AFX of eyelid with canalicular involvement in a 43-year-old woman who initially presented as a chalazion.Case presentation: The patient is 43-year-old woman with a large mass since 4 years ago in the lower eyelid of her right eye adjacent to medialcanthus with lower canalicular involvement with the early diagnosis of chalazion 3 years ago. During this period she underwent surgery because ofmass recurrence and the mass was removed with a safety margin of 2cm then biopsy specimen was sent for histopathology and immunohistochemistry. Because of the mass safety margin and proximity to the lower canaliculus, anterior wall of lower canaliculus excision was performed. Histopathology and immunohistochemistry revealed AFX diagnosis. Conclusions: Although AFX rarely occurs on the eyelid, it shouldbe considered due to clinical similarity in cases of recurrent chalazion in the eyelid. Also it should be considered as a differential diagnosis in eyelidtumors especially malignant forms in cases chalazion has exceeded from tarsal plate with other tissues involvement such as canaliculus
