Background and Objective: Cancer cells secrete more proteins, lipids and nucleic acids compared to normal cells. These molecules are transferred by exosome (30-100 nm), a lipid bilayer vesicle, to other cells. Exosomes have important role in tumor cell communication with their environment and are involved in pathogenesis, development, progression, and metastasis of cancer. Exosomes may determine the organ specificity in metastasis and lead to metastasis by several approaches including angiogenesis, immune modulation and microenvironment preparation. Therefore, inhibition of the production, secretion and transportation of exosomes may serve as an effective treatment for inhibiting cancer metastasis. In this study, we assess scientific evidence related to the efficacy of these treatments.Search Method: PubMed, Scopus and Google Scholar were searched from January 2013 to December 2017. Based on inclusion and exclusion criteria ten relevant experimental studies with moderate bias were selected. The effect of exosome inhibition on preventing cancer metastasis was evaluated according to the number of metastatic foci (metastatic rate) and the largest metastasis size. Findings: Our analysis showed that the presently available evidence confirmed the therapeutic potential of exosome inhibiting in significantly reduction metastasis size and frequency to other organs. Conclusions: We suggest a new treatment plan for preventing metastasis of a wide variety of tumors via the suppressing of the pro-metastatic functions of cancer-derived exosomes, but many extensive clinical studies are required in the future to investigate the features and safety of tumor exosome mediated therapies. Also, we suggest in future studies the ability of exosomes to use as a specific nanocarrier for targeted drug delivery of selective DNAs, miRNAs, non-coding RNAs, and proteins to cancer cells be investigated.
