Shell-Tube Heat Exchangers and Plate Heat Exchangers, as heat transfer equipment, have extensive applications in oil, gas, refinery and petrochemical industries. In this novel and applicable article, for the first time, it was studied identification, analyses, assessment and control of risks of layer deposition, corrosion and erosion of plate & shell-tube heat exchangers by FMEA methodology. Results showed that temperature, salt concentration, salt PH, salt velocity, flow turbulences, surface flatness, distribution of fluid, water hardness and dead spots in heat exchangers were major parameters for risk assessment. Also concentrations of Chlorine & Fluoride in the fluid stream and Ammonia existence in the fluid stream are very important in risk assessment of corrosion of plate heat exchangers. In addition to, velocities of different streams were very vital in risk assessment of erosion in plate heat exchangers. Results of risk numbers calculations before control actions showed after applying control actions, all risk numbers decreased 75%, that this decrease was significant. This decrease showed that all of control actions were appropriate and effectiveness. RESEARCH HIGHLIGHTS 1) For the first time, risk assessment and risk analysis of plate & shell-tube heat exchangers for cases of layer deposition, corrosion and erosion was studied. 2) For the first time, FMEA method was applied for this purpose. 3) Effective control actions were suggested for decreasing risks.
