Evaluation of the relationship between maternal serum PAPP-A and β-hCG levels of the first trimester of pregnancy and fetal macrosomia in singleton pregnancies

Objectives: Although it has been reported that serum levels of first trimester screening markers are associated with birth weight, the results of previous studies are contradictory. Methods: Maternal data, first trimester screening data (PAPP-A level, β-hCG level and CRL), and gestational age at birth of 856 women with singleton pregnancy were recorded. Infants were divided by gestational age at birth (for weeks 37 to 40) with a weight greater than 90th percentile as fetal macrosomia, below 10th percentile as low birth weight (LBW), and between 10th and 90th percentiles as normal babies.Results: Sixty women (7%) had diabetes and 13 persons (1.5%) had hypertension. 86 neonates (10%) were macrosomic, 695 neonates were (81.2%) normal and 75 neonates (8.8%) were LBW. Univariate analysis showed that maternal BMI in fetal macrosomia was significantly higher than normal and LBW neonates (P=0.005) and maternal parity in LBW neonates was significantly lower than macrosomic or normal babies (P=0.020). There was no significant difference among neonatal status in terms of other variables(P> 0.05). There was no significant difference between the diabetic and non-diabetic mothers in terms of neonatal status(P=0.269). However, in mothers with hypertension, the frequency of LBW neonates was almost two-times and the frequency of macrosomia was almost three-times more than normal neonates(P=0.023). The results of multivariate logistic regression analysis showed that macrosomia predictors were BMI, maternal hypertension and CRL.Conclusions for Practice: The findings showed that there was no significant difference among macrosomic, normal, and LBW neonates in terms of the first trimester screening markers
