Evaluation of Relation of CRI and HRR on Exercise Test with Coronary Artery Disease in Angiography Candidates

Background and objective: Coronary angiography is an invasive and costly method for determining the severity of coronary involvement. The type and number of vessels caught, havean important role in the rate of myocardial ischemia and therefore in the severity of the symptoms of the disease and prognosis. Knowing the severity of CAD before starting treatment and using a non-invasive method such as exercise testing is helpful in prioritizing patients for choosing the right type of treatment and emergency intervention and can increase accuracy and speed in the treatment process. Therefore, this study was performed to determine the predictive potential of CRI and HRR1min recovery and HRR2min recovery from exercise testing. Materials and Methods: This cross-sectional study was conducted on 160 patients undergoing angiography candidate referring to Ali ibn Abi Talib Hospital in Zahedan, which had positive exercise test in 95-96 years. Sampling was done by census method. The results of the angiography and the indices obtained from the exercise test (CRI, HRR1, HRR2) were entered in the information form of the patients with the entry conditions. Then the collected data was analyzed by SPSS. Differences in ratios were investigated using chi-square test. logistic Regression test was used to analyze the variables obtained from the exercise test.Findings: Sensitivity, specificity, positive predictive value & negative predictive value of CRI to predict the severity of coronary artery disease is respectively 15%, 70.2%, 50%, 70%.This is about HRR1 , respectively, 29%, 85%,85%,40%.These parameters for HRR2 are respectively 70%, 56%,70%,45%. There was a significant relationship between the severity of coronary involvement with age, However, there was no statistically significant correlation between severity of coronary artery disease with any of the parameters of sex, diabetes, high blood pressure and smoking. There was a significant relationship between the severity of coronary artery disease and each of the CRI, HRR 1 and HRR 2 indices separately. Although, using multiple logistic regression analysis methods, independent variables were not predictive of coronary artery disease in the presence of the present.Conclusions: The three CRI, HRR 1 and HRR 2 indices from the exercise test alone can be a predictor of the severity of coronary artery disease. Considering the small number of patients present in this study and the importance of the subject, it is suggested that studies at a wider level and with more details, such as a closer examination of coronary artery disease in more distinct groups in terms of severity of conflict.
