An Unusual Case of Fenpyroximate Poisoning, Probably Mixed with Organophosphate

Introduction: fenpyroximate is a mitochondrial NADH-ubiquinone oxidoreductase inhibitor. It is used in the agricultural sector as an acaricide. There are few case reports of poisoning with fenpyroximate.Case report: a 29 years old man brought to the hospital following ingestion of 15 tablets of alprazolam and 15 tablets of valprovate sodium and 120 ml of Fenpyroximate 5%. The patient had sialorrhea, lacrimation and diarrhea, systolic blood pressure was 60 mmHg, heart rate: 120, venous blood gas analysis showed metabolic acidosis. Atropine was administrated considering cholinergic signs of patient, after 14 hours cholinergic signs diminished and atropine administration was discontinued .One day after admission patients body temperature raised, ceftriaxone and clindamycin were administrated for him, after 9 days hospitalization the patient discharged and referred to psychiatrist.Discussion: fenpyroximate toxicity signs include: nausea, vomiting, incoordination, convulsions, central nervous depression and respiratory distress, bradycardia and arrhythmia but our patient surprisingly had cholinergic signs that could be consequence of mixing fenpyroximate with organophosphate to get better efficacy so mix toxicity of pesticides should be consider by clinicians to proper diagnosis and management of pesticide poisoning.
