Background: In modern societies, tobacco uses is very common and affect the various body systems. The aim of this study was to determine the relationship between cigarettes and other tobacco products with infertility in men and women.Methods: In a systematic survey of the databases such as PubMed, Elsevier, Ovid, Clinical key, SID, IranMedex and etc. from 2005 to the present, the relevant articles were extracted and analyzed.Result: The results showed that tobacco use by increasing oxidative stress and DNA damage of cells in semen fluid decreases the sperm production stimulates sperm; lower the natural forms and fertility ability of sperms. Also, smoking increases the rate of infertility in women. There is a direct relation between smoking duration and intensity of infertility. By contrast, smoking cessation, weight loss and iron supplementation improved the treatment of infertility.Conclusion: The results showed that long-term tobacco use can lead to infertility. Therefore, advise against smoking can solve one of the major risk factors for infertility and quitting smoking can prevent the progression and exacerbation of infertility
