New issues for diagnostic evaluation and treatment of ABO incompatibility in newborn

Background and Aim: ABO and RH incompatibility are the most common causes of newborn hemolytic disease(HDN that appears within 48 hours after Birth.The purpose of this study is to present and highlight the recent findings about diagnosing and treatment of ABOincompatibility.Methods:This article was composed by reviewing recent reports indexed by the authoritative sections of Google scholar, Pubmed and up to date, using the keywords ABO,IVIG,JK3,COHB since2016.Conclusion:Results from reviewing articles for latest findings for diagnosing HDN shows: 1-Analysis of pre-natal antibodies by umbilical cord sample with Eliza method shows a significant relationship between antibody of mother s blood group and bilirubin rise in cord blood2.In HDN with thrombosis accompanied with cytopenia, evaluation of serum erythropoietin, folate and vitamin E is advised 3-Patients with Hyperbilirubinemia caused by G6PD deficiency or ABO or RH incompatibility who received IVIG showed reduction in number of exchange transfusion, phototherapy and length of admission in comparison with those who had not received IVIG.4.COHB values measured with CO-Oximeter among jaundiced babies due to hemolysis were higher than non-hemolytic cases whereas values of COHB titer as1.7%, shows sensitivity and specificity of 72% and 97% for detecting ABO incompatibility, respectively.5.A Chinese study reported the effectiveness of oral administration of herbal medicine drug (CHM) in mother during pregnancy may prevent HDN due to ABO incompatibility.6.Pre-biotic consumption is advised for reducing Hyperbilirubinemia.7.In ABO s incompatibility, the possibility of successful heart transplantation is low.8.Although the serum total Jk3 antibody titer does not correlate with severity of hemolytic disease, it is recommended that the titer of16-32 is accepted as indicating point for high risk HDN.9.Among ABO incompatible cases with positive coombs and black race, type BO blood group is more accompanied with hemolytic anemia and Hyperbilirubinemia.10.Evaluation of molecular DNA within pregnancy can display blood group Antigene and is provided for patients who did not have a history of blood transfusion.
